Nursing Assistant Related Instruction

The Southwest Tech Nursing Assistant Program is approved by the Wisconsin Department of Health and Family Services, preparing the student to be successful in meeting state and federally regulated competencies as a nursing assistant. The graduating student is eligible to take the National Nurse Aid Assessment Program Exam, which includes both a written and skills exam.

The Nursing Assistant courses will now have hard registration deadlines.

The next course will take place from March 18 - May 17 and the registration deadline is March 6 at 4:30 pm. This means that a student must apply and complete requirements by March 6.

Below is the information that needs to be completed and sent to Gin Reynolds at greynolds@swtc.edu to meet the course acceptance requirements:

• Background disclosure form
• Functional ability form
• A physical - click here for the physical form
• 2 step TB skin test - click here for the form (The tb test may take some time as it is a 2-step, but as long as they submit everything else and start the tb test, then Gin may let them provisionally register for the class.)
• Proof of immunizations - this can be provided by your primary care provider or from the Wisconsin Immunization Registry

Students who have been accepted to this course beginning on March 18 (already applied and completed all requirements), should email admissions@swtc.edu to let them know that they will be participating in the class.

Please give this letter to your Youth Apprenticeship students who will be taking the CNA course at Southwest Tech.
